Immunization day set for Grand Ridge Middle School

Attention parents: On May 20, the Jackson County School Board will offer the parents of current sixth-grade students at Grand Ridge Middle School an opportunity to obtain their child’s required (2008/2009) seventh-grade immunizations on school campus. This special service is a joint effort between the Jackson County School Board and the Jackson County Health Department. The Jackson County School Health program nurses will be administering the vaccinations.
Immunization records in the Jackson County school computer data base have been compared with the new “FLORIDA SHOTS” system to make sure that all possible vaccine dates are noted before vaccinations are given.
Parents should have received an immunization packet on May 14 brought home by his/her child from Grand Ridge Middle School. This packet includes vaccine information statements, privacy information, and an “authorization to immunize” form. The “authorization to immunize” form must be returned to Grand Ridge Middle School by Thursday, May 15th for a child to receive vaccinations on May 20th. Parents are welcome on campus these days, but presence is not required as long as and “authorization to immunize form’” is signed and returned. Parents, please assist us in having great success with these days.
